ARMD Management

Medicinal In most cases, macular degeneration, which is also called age-related macular degeneration or AMD, produces slow and completely painless vision loss over a period of years. This vision loss develops as the macula of the eye becomes damaged.

Macula is located at the center of your retina.

If you are over 40 years of age, you have probably noticed changes in your vision. Difficulty seeing clearly for reading and close work is among the most common problems adults develop between ages 41 to 60.

A healthy macula enables you to see objects with greater detail. As the macula deteriorates, you could develop fuzziness or blind spots in the center of your field of vision.

If straight lines appear distorted or wavy or there appears to be a blind spot or empty area in the center of your vision, you may have the signs of age-related macular degeneration (AMD).

  • Spots, which may be dark or light, appear in your center vision.
  • Straight lines begin to appear warped or wavy.
  • You experience changes in the sharpness of your center vision

Pre & Post Operative Management

Medicinal Post Operative Endophthalmitis is a devastating and serious complication of Intraocular Surgery. Fortunately, in recent times, due to better surgical techniques and improved sterilization and aseptic methods, the incidence has come down to very low levels. However, sometimes despite all precautions, infections do occur. The risk factors for developing postoperative endophthalmitis are multifactorial.

Proper lid hygiene should be maintained in the postop period

Topical fluoroquinolone antibiotics 4-6 times per day should be administered for 7 to 10 days along with topical steroids in tapering doses for 6 to 8 weeks. Cycloplegics if required as prescribed by the surgical team.

All patients should be given a discharge card mentioning the surgical details and postoperative instructions including date for follow up examination and location.

Regular follow up of the patients is necessary. Patients should be referred to a referral Centre Immediately upon early symptoms (e.g. pain, redness, dimness of vision, discharge) and signs (e.g. circumcorneal congestion, anterior chamber cells and flare, hypopyon, reduced fundal glow, vitreous exudates) of infection/endophthalmitis.

Personal hygiene to emphasized.

Avoid dust, smoke and sunlight.

Wear protective dark glasses outdoors.

Frequency of instillation of Eye drops as advised to be ensured.

Allergy Management

When your eyes are exposed to substances like pollen or mold spores, they may become red, itchy, and watery. These are symptoms of allergic conjunctivitis. Allergic conjunctivitis is an eye inflammation caused by an allergic reaction to substances like pollen or mold spores.

The inside of your eyelids and the covering of your eyeball have a membrane called the conjunctiva. The conjunctiva is susceptible to irritation from allergens, especially during hay fever season. Allergic conjunctivitis is quite common. It’s your body’s reaction to substances it considers potentially harmful.

Don't overlook itchy, red, and swollen eyes – they may be symptoms of an eye allergy.

The usual suspects - Pollen, Dust mites, Pet dander, feathers & other indoor or outdoor allergens can set off eye allergy symptoms.

Symptoms of eye allergies include

Red, irritated eyes , Itchiness , Tearing or runny eyes , Swollen eyelids , Soreness, burning, or pain

Glaucoma Management

Medicinal Glaucoma is a group of related eye disorders that cause damage to the optic nerve that carries information from the eye to the brain.

In its early stages, glaucoma usually has no symptoms, which is what makes it so dangerous — by the time you notice problems with your sight, the disease has progressed to the point that irreversible vision loss has already occurred and additional loss may be difficult to stop.

In most cases, glaucoma is associated with higher-than-normal pressure inside the eye — a condition called ocular hypertension. But it also can occur when intraocular pressure (IOP) is normal. If untreated or uncontrolled, glaucoma first causes peripheral vision loss and eventually can lead to blindness.

Glaucoma often is called the "silent thief of sight," because most types typically cause no pain and produce no symptoms until noticeable vision loss occurs.

Treatment can involve glaucoma surgery, lasers or medication, depending on the severity.

Eye drops with medication aimed at lowering IOP usually are tried first to control glaucoma.

Because glaucoma often is painless, people may become careless about strict use of eye drops that can control eye pressure and help prevent permanent eye damage.

Medicinal Antibiotic Range
Antibiotics are among the most frequently prescribed medications in modern medicine. Antibiotics cure disease by killing or injuring bacteria. The first antibiotic was penicillin, discovered accidentally from a mold culture. Today, over 100 different antibiotics are available to cure minor, and life-threatening infections.

Although antibiotics are useful in a wide variety of infections, it is important to realize that antibiotics only treat bacterial infections.

Gastrointestinal Range

The gastrointestinal (GI) system is made up of the GI tract plus accessory organs. In essence, the GI tract is a long hollow tube that extends from your oral cavity where food enters your body, via the esophagus, stomach, small intestine, large intestine, rectum, and finally to the anus where undigested food is expelled. The accessory organs include the salivary glands, pancreas and liver. These secrete important enzymes into the digestive tract. The gall bladder, which stores bile, is also considered part of the GI system.

Some of the most common GI problems are constipation, diarrhea, nausea and vomiting. These may be mild, self-limiting and temporary or may persist over the long term and affect your health. For instance, sustained constipation can lead to diverticular disease (where the intestine becomes damaged from passing hard stools and then form pocket-like sacs) and impaction (causing the intestine and rectum to become blocked). Sustained diarrhea can make your blood pressure drop, which can lead to fainting or heart rhythm abnormalities. Nutrient and fluid loss from your body due to diarrhea or vomiting can lead to dehydration, electrolyte imbalances, vitamin deficiencies, weight loss and malnutrition. Similarly, nausea, indigestion and constipation can reduce your appetite, and thus reduce your food intake leading to nutritional deficits. The nutritional effects of GI problems can be particularly serious for people who are already malnourished or ill.

Gastroesophageal reflux disease (GERD) occurs when stomach acid frequently flows back into the tube connecting your mouth and stomach (esophagus). This backwash (acid reflux) can irritate the lining of your esophagus.

Many people experience acid reflux from time to time. GERD is mild acid reflux that occurs at least twice a week, or moderate to severe acid reflux that occurs at least once a week.

Pain Management

The treatment of pain is guided by the history of the pain, its intensity, duration, aggravating and relieving conditions, and structures involved in causing the pain. In order for a structure to cause pain, it must have a nerve supply, be susceptible to injury, and stimulation of the structure should cause pain.

Oral & Throat Infections

The oral cavity contains some of the most varied and vast flora in the entire human body and is the main entrance for two systems vital to human function and physiology, the gastrointestinal and respiratory systems. Several diseases involve these two systems and manifest in the oral cavity. In addition, a specific pathologic condition, such as periodontitis (ie, inflammation of the periodontal attachment of the teeth and the alveolar bone), may be present in the oral cavity.

A variety of organisms in the microenvironment of the oral cavity adhere to the teeth, the gingival sulcus, the tongue, and the buccal mucosa. Each site has a unique way of allowing the organisms to establish their residency. The normal flora in healthy individuals maintains similar patterns. When a local or systemic disease process or concomitant use of medications alters this overall pattern, atypical organisms begin to predominate and some normal organisms with a benign nature, such as Candida albicans, become pathogenic.

Chlorhexidinegluconate oral rinse, 0.12% provides antimicrobial activity during oral rinsing. The clinical significance of 0.12% Microbiological sampling of plaque has shown a general reduction of counts of certain assayed bacteria, both aerobic and anaerobic, ranging from 54-97% through six months use.

Use of Chlorhexidinegluconate oral rinse in a six month clinical study did not result in any significant changes in bacteria resistance, overgrowth of potentially opportunistic organisms or other adverse changes in the oral microbial ecosystem.
